Purchase orders are accepted from schools, clinical institutions, state/government, and non-profit organizations. Invoices are billed net 30 day terms.

Subtotal must be at least $199 for all purchase orders.

The following information must be included: PO number or reference, contact name, billing and shipping addresses, product information, quantities, price, and signature. Purchase orders originating in Nevada and/or Arizona are subject to sales taxes.
